Fire Claim Checklist

  • Letter of Intimation of Loss.
  • Fire Brigade Report along with the forwarding letter/copy.

  • Certificate issued by the Chairman/Ward Commissioner/Meteorological Department (applicable in cases of flood and cyclone).

  • Incoming and outgoing stock register for the six months preceding the incident (applicable for warehouses/godowns).
  • Copy of the electricity connection approval letter (where applicable).
  • Certified copies of electricity bills for the last three months.
  • Valid permission/license for self-generated electricity (if applicable).
  • Documents confirming that adequate electrical safety measures were in place to prevent short circuits.
  • Copy of the Fire License.
  • Shipping documents for imported machinery/materials.
  • Building layout plan (applicable for mills/factories).
  • Electrical layout plan (applicable for mills/factories).
  • Periodical test reports for the six months preceding the incident (applicable for mills/factories).
  • Details of fire-fighting equipment.
  • Repair quotations/invoices for building or machinery (where applicable).
  • Newspaper clipping relating to the incident.
  • Claim bill showing the quantity, value, and total amount claimed for the damaged goods.
  • Any other supporting documents (where applicable).

Motor Claim Checklist

  • Letter of Intimation of Loss submitted within the prescribed time.
  • Copy of the Insurance Certificate/Policy.
  • Copy of the General Diary (GD) Entry/FIR.
  • Driver's statement.
  • Vehicle Registration Certificate/Blue Book.
  • Fitness Certificate.
  • Tax Token.
  • Driving License duly verified by BRTA.
  • Route Permit (for commercial vehicles).
  • Three repair quotations from three different workshops.
  • Certified copy of the Police Report (for third-party liability and theft claims).
  • Form "K" (for Bodily Injury claims).
  • Form "K" and Succession Certificate (for Death Claims).
  • Copy of the application submitted to BRTA for suspension/blocking of the registration number (for theft claims).
  • Any other supporting documents, if required.

Marine Cargo Claim Checklist

  • Copy of the Insurance Policy.
  • Copy of the Bill of Lading/Air Waybill (both sides/pages).
  • Copy of the Invoice.
  • Copy of the Packing List.
  • Copy of the Bill of Entry.
  • Copy of the Letter of Credit (L/C).
  • Non-Delivery Certificate (applicable in case of non-delivery claims).
  • Copy of GD Entry (for theft, burglary, or missing cargo claims).
  • Fire Brigade Report (for fire-related claims).
  • Copies of correspondence regarding recovery of compensation from third parties.
  • Marine Note of Protest (if necessary).
  • Letter of Subrogation/Letter of Indemnity (where applicable).
  • Claim bill indicating the quantity, value, and total amount claimed for the damaged goods.

  • Notarized Affidavit of the Master/Driver.
  • Copy of the Vessel Registration Certificate.
  • Marine Note of Protest.
  • Certificate of Inspection.
  • Government-issued Fishing License (applicable for fishing trawlers).
  • Three repair quotations from different repair yards/workshops.
  • Competency Certificate of the Master/Driver.
  • Casualty Report as per the Inland Shipping Ordinance, 1976.

Miscellaneous Claim Checklist

  • Letter of Intimation of Loss.

  • Hospital bills, medicine purchase receipts, and pathology reports (for hospitalization claims).
  • Medical Certificate and Post-Mortem Report (for accidental death claims).
  • Copy of the Succession Certificate (for accidental death claims).
